A. Purpose of the Role
The Part-time Support Worker primarily delivers person-centered support services. Support procedures and practices necessary to facilitate the Association’s ability to fulfill its vision and values. As part of the staff team, assist in the development, implementation, assessment andrecord maintenance of Individual Service Plans, behaviour management programs and supports. Ensure the health and safety of individuals by assisting them in preparing nutritious meals and maintaining a secure, clean, neat, safe, and well-maintained home environment bymodelling appropriate behaviors.

C. Specific Role Accountabilities
(These divide the purpose of the role into key elements. They should be in enough detail to provide clarity on what the individual will be called to account for on the role. Accountabilities are not time-bound, or as detailed as goals, objectives, or task lists.)1. PlanningUnder the leadership of the Manager, participates in the development of an annual team charter.
i. Action, monitor and provide feedback to the team to achieve the goals consistent with the Agency's vision and values.
2. Quality AssuranceProvide and enhance person centered supports in accordance with the Agency’s policies and procedures regarding quality assurance.
i. Support the completion of Individual Support Plans (ISP) and agreements by providing accurate documentation guided by the Agency’s values enhancing quality of life for people.ii. Use all communication modes to build relationships based on equality, integrity, and interdependence.iii. Follow the Agency’s policies and procedures on abuse ensuring education, prevention, and reporting.iv. Respect confidentiality and privacy of people by safeguarding all information and use solely for the purpose of service obligations.
3. Relationships and NetworkingCultivate and maintain positive, effective working relationships with people with disabilities, families/friends, team, and communities which exemplify the vision and values of the Agency. i. Work in alliance with staff members, volunteers, and students to ensure people are welcomed and valued in all communities.4. Supports and Service DeliveryAs directed, complete daily, weekly, and monthly tasks in a timely manner. Ensure emotional, physical, spiritual, social/leisure and vocational well-being of people. Support their chosen lifestyle by promoting social inclusion, community participation and natural supports.
i. Support the individual to engage in meaningful life activities by supporting the development of skills in daily living, self-help, health, communication, social and spiritual interaction, and community awareness through education and modeling such as:
• Personal care, hygiene• Meal preparation, nutrition• Administer medication as per ISP and Agency policies, promote good health• Teach money management and budgeting skills• Accompany and/or transport people within the community - shopping, leisure or recreational activities, appointments
ii. Log entries are expected to be read at the beginning of shift. Written documentation is to be clear, concise, and factual within all case notes, log reports and correspondence.iii. Ensure all interactions are respectful and align with People First Language.iv. Act with consideration for the person’s cultural beliefs, interests, choices, and rights.v. Upkeep environment through regular cleaning and household chores.
5. Team SupportActively contribute to team success and growth.
i. Actively participate as a team member in the development, implementation, and evaluation of supports.ii. Attend and actively participate in regularly scheduled staff meetings.iii. Participate in the orientation and evaluation of new employees, students and volunteers.iv. Further develop personal and professional growth by participating in research, workshops, seminars, and relevant committees.
6. Health & SafetyHave a clear understanding of employee/worker responsibilities as identified in the Occupational Health & Safety Act and in the Agency’s practices and procedures.
i. Take personal responsibility for minimizing risk to self, coworkers, students and volunteers through orientation, education, and action.ii. Maintain the physical environment to ensure secure, neat, clean, and safe.iii. Conduct health & safety inspections, corrects hazardous acts and/or conditions, identify maintenance needs and make recommendations.iv. Maintain equipment and adaptive assistive devices.v. Perform fire drills and document according to established procedures.vi. Provide appropriate intervention in crisis according to established procedures.vii. Responsible to report and document any workplace injury, accident/incident or illness.viii. Participate in workplace risk assessment.ix. Cooperate and participate in Return-to-Work plansx. Appropriately use or wear protective equipment, devices or clothing as directed by the Agency.xi. Perform safe driving practices and follow the guidelines for Agency owned vehicles.xii. Participate within Health & Safety information session and/or training.
D. Educational & Experience Requirements
• Developmental Services Worker (DSW) or Behavioral Science Technician (BST) or Social Services Worker (SSW); with Pharmacology or equivalent education with pertinent experience.• Current valid Certificates – First Aid/CPR, NVCI and Driver’s License & Abstract• Experience working with adults with disabilities• Excellent interpersonal communication• Understands, demonstrates, and is committed to working with people adhering to the Agency’s’ “Service Values” & “Bill of Rights”• Ability to work effectively as a team member and in collaboration with outside agencies.
E. Knowledge & Skills Requirements
• Excellent and effective written, verbal and listening communication skills.• Effective interpersonal and public relations skills.• Computer literacy of a level sufficient to document supports and services and meet administrative needs.• Collaborative, facilitative, confident, consistent yet flexible decision-making skills.• Resourcefulness and flexibility to meet the needs of the Agency and demands of the position.• Ability to work independently and as a team member to achieve outcomes and deadlines.• Ability to balance multiple demands/priorities and successfully carry out the vision, values and priorities of person centered supports and services.• Working knowledge of applicable legislations, regulations, contracts:

H. Employment & Working Conditions
Specific employment and working conditions are specified in organizational policy include but are not limited to the following:

• Work is generally performed in a home and/ or a community setting and includes exposure to occasional loud levels of noise; frequent distractions and interruptions, pressure from multiple demands and occasionally short deadlines and exposure to eye strain from technology devices.• Ability to work a flexible schedule.• Physical/cognitive demands are representative of those that must be met by an employee tosuccessfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.• While performing the duties of this job, the employee is regularly required to use hands to manipulate, handle or feel. The employee is regularly required to stand, walk, and reach with hands and arms. The position requires some driving. The employee is regularly required to lift and/or move supplies/equipment/furniture up to 10 pounds and may be up to 25 pounds. Specific vision abilities required by this job include vision, distance vision and peripheral vision. The position requires ability to read regular print. Vision requirements for driving must be met.• Process verbal and written material, using measuring and mechanical devices, problem-solving, good judgement, and common sense.• The employee is required to provide cognitive and physical support to individuals (i.e., personal care), maintenance/housekeeping activities and must be physically able to provide intervention in crises situations.
